OFFICE UPDATE: Dr. Storseth is retiring from Family Practice effective 9.30.23. Established patient of hers are welcome to continue their medical care with the providers at Family Practice Associates, and there is no need to request medical records at this time. Please contact our office at your convenience to make any necessary appointments, request refills or address any medical concerns you may have.

 

If you prefer to transfer care to an outside provider, you can request copies of your records be released and sent to your new provider either by calling our office, or in person.
